Yes, it is possible to customize a Form Template by creating a duplicate of a System Default template within the Template Editor.
The Template Editor can be found by navigating to (Form Content & Templates > Template Editor > Forms - Core View & DFMEA).
Note: Data created under one Form Type may not carry over when switching to a new custom Form template.
